This site is privately operated. Mentions of DOH, HIICAP/NYSOFA, New York Medicaid Choice, or Medicare.gov are for your convenience and do not mean we are endorsed by or part of those agencies, except that HIICAP is a free public counseling resource we encourage you to use: 1-800-701-0501.
No enrollment promises
Using the dual-plan help tool at /tools/dual-plan-help or submitting a form does not enroll you in, or disenroll you from, any D-SNP, IB-Dual arrangement, MAP plan, or Medicaid coverage. Default enrollment and opt-out are controlled by your official notice and the plan / state / CMS process.
