Live outside the five boroughs?
The same programs exist in Nassau, Suffolk, Westchester, Rockland, and the rest of the state, but Medicaid there runs through your county Department of Social Services rather than HRA, and not every county has every integrated plan option. Call the free line below and say your county name; you will be routed to your local program. County pages are planned for a later update.
