Mid-Island: D-SNPs, IB-Dual, and the “default enrollment” letter

Mid-Island is the wide middle of Staten Island: Willowbrook, New Springville, and Bulls Head to the west, New Dorp, Dongan Hills, and Grasmere toward the water, and Todt Hill in between. The housing is mostly one- and two-family homes, and the people getting default enrollment letters here are often older homeowners who added Medicaid to Medicare after a spouse’s illness, or adults on disability living with family.

This page is for those households and for the adult children who manage the paperwork from the kitchen table. It explains what the plan’s letter is asking, what IB-Dual means, when Medicaid Advantage Plus comes into the picture, and how to get a free counselor from Mid-Island. It is not a government page and does not enroll anyone.

Mid-Island is in Richmond County. That is the name Medicare and Medicaid use for all of Staten Island, so a county field wants “Richmond,” even though your mail says Staten Island, NY. Medicaid is administered by HRA citywide. ZIP 10314 covers Willowbrook, New Springville, Bulls Head, and Westerleigh; 10306 covers New Dorp, Oakwood, and Richmondtown; 10305 covers Dongan Hills, Grasmere, and Arrochar (approximate; verify with USPS).

Staten Island University Hospital’s North campus, on Seaview Avenue in Ocean Breeze, is the Mid-Island hospital, with affiliated practices along Hylan Boulevard and Richmond Road, each either in a given D-SNP’s Medicare network or not. The Staten Island Railway stops at Grasmere and New Dorp on its way to St. George, where the island’s only in-person HIICAP site is; on the west side, the Staten Island Mall in New Springville is the bus hub.

In Mid-Island the letter usually lands in a single-family mailbox rather than a lobby, so it rarely gets lost; it gets set aside. If your Medicaid managed care plan also runs a Medicare D-SNP, the notice comes from the plan at least 60 days before Medicare starts. Not every plan sends one. Find the effective date and the opt-out deadline on the letter, not from memory.

The Mid-Island-specific step is a Hylan Boulevard check. Many families here use a primary care office on Hylan or Richmond Road, a specialist attached to Staten Island University Hospital North, and a pharmacy near New Dorp Lane. Look each one up in the D-SNP’s Medicare and Part D directories before the effective date, and call HIICAP that week if one is missing.
