According to the New York State Department of Health, a D-SNP includes doctor, hospital, and prescription drug coverage. Some plans may also offer extra benefits such as transportation, home-delivered meals, or over-the-counter (OTC) allowances. Exact extras vary by plan and year.
Who can join (high level)
In general, you may be able to enroll in a D-SNP if you:
- Have Medicare (typically Part A and Part B), and
- Have Medicaid benefits (full Medicaid or certain levels of help), and
- Live in the plan’s service area
Only an official enrollment process can confirm whether a specific plan will accept you. This article does not say you qualify for any plan.
Why people look at D-SNPs
Having Medicare and Medicaid at the same time can mean two sets of rules, cards, and phone numbers. A D-SNP is designed so one health plan helps coordinate Medicare benefits, and, depending on the arrangement in New York, work more closely with your Medicaid coverage.
Coordination does not automatically mean every doctor you see today is in the new network, or that every benefit stays the same. Always compare the plan’s Evidence of Coverage and provider directory, or ask a counselor to help you check.
D-SNPs and other New York options (brief)
New York describes several ways dual eligibles may get more integrated care, including D-SNPs and programs such as IB-Dual (for many people who do not need long-term community-based services beyond certain limits) and Medicaid Advantage Plus (MAP) (for some people who need longer-term care supports). Which path fits depends on health needs, county, and current Medicaid enrollment, not on a website quiz.

People who are dually eligible often have more than one chance in a year to change certain Medicare coverage. Rules change; confirm current Special Enrollment Periods with Medicare, your plan, New York Medicaid Choice, or HIICAP.
