Checked against the source: Medicare.gov lists 1-800-MEDICARE (1-800-633-4227), 24 hours a day, 7 days a week except some federal holidays. New York's Office for the Aging lists HIICAP at 1-800-701-0501 for free Medicare counseling. Plan member-services numbers are on your current plan ID card, not on a generic "D-SNP Medicare phone number" list.
Last checked September 28, 2026. Plan phone numbers can change every year, so an old number from a 2021 or 2023 letter may not work now.
D-SNP Medicare phone number: find your plan's line
- Look at the back of your plan ID card. Member services is usually listed there, often with a TTY number and a separate pharmacy line.
- Check your plan mail. Your welcome letter, Evidence of Coverage, and the Annual Notice of Change that plans send each fall all list member services.
- Look it up on Medicare Plan Finder. Go to medicare.gov/plan-compare, enter your ZIP code, and find your plan by name. The plan's contact information is on its details page.
- Not sure which plan you are in? Call 1-800-MEDICARE with your Medicare number. They can tell you your current Medicare plan.
- Got a default enrollment letter? The plan's number is on the letter. You can also ask HIICAP to go over it with you before you decide anything. See our guide to the first 7 days after a default enrollment letter.

Why the "phone number 2021" answer changed
People often search for a D-SNP phone number with a year in it, because the number came from an old letter. Plans can change names, phone lines, and even companies from one year to the next. New York's own chart of integrated plans for dual eligible New Yorkers is updated each year on the Department of Health's website. Always use the number on your current card or this year's plan mail.
Watch out for callers who say they are "Medicare"
Federal rules say Medicare Advantage plans and agents may not make unsolicited calls to you. They can call only if you asked them to. If someone calls out of the blue and says you must switch plans, or asks for your Medicare number, hang up. Call the number on your card or 1-800-MEDICARE yourself.

Common mistakes
- Calling a number from an old letter or an ad instead of the one on your current card.
- Calling your Medicaid office about Medicare plan benefits. Your D-SNP or 1-800-MEDICARE handles those.
- Giving your Medicare number to someone who called you.
- Switching plans over the phone before checking that your doctors and drugs are covered.
