Pelham Parkway: D-SNPs, IB-Dual, and the “default enrollment” letter

Pelham Parkway is lined with six-story apartment buildings from Bronx Park East to Williamsbridge Road, and many residents have lived there for decades. A lot already have Medicare and Medicaid together, and a meaningful number receive home care through Medicaid. Others are just now aging into Medicare from a Medicaid managed care plan and have a letter on the kitchen table that mentions a D-SNP.

This page is for both groups and for the relatives and home attendants helping them. It covers what the letter asks, why there are two versions of it, how the home care question changes the answer, and where the nearest free counselor sits. It is educational only and does not enroll anyone.

Pelham Parkway is in Bronx County, and Medicaid here is run by HRA citywide, not by a county office. The area is usually described with ZIP 10462 south of the parkway toward Parkchester, 10461 east toward Morris Park, and 10467 north toward Allerton and Williamsbridge (approximate; verify with USPS). If a form asks for county, the answer is “Bronx.”

Two hospitals sit a short bus ride apart on the south side of the parkway: NYC Health + Hospitals / Jacobi on Pelham Parkway South, a public hospital, and Montefiore Einstein’s Jack D. Weiler Hospital on Eastchester Road in Morris Park. A single D-SNP may include one, both, or neither in its Medicare network. The 2 and 5 trains stop at Pelham Parkway, and the borough’s closest in-person HIICAP site, at the R.A.I.N. senior center on Metropolitan Avenue in Parkchester, is a short bus trip south.

Pelham Parkway has an unusual number of households where one letter is the non-long-term-services version and the neighbor’s is the long-term-services version. If you are not expected to need more than 120 days of community-based long-term services, the letter generally points to IB-Dual, which means staying in your Medicaid managed care plan alongside the aligned D-SNP. If you need 120 days or more, it generally points to a Medicaid Advantage Plus plan instead. Read the first paragraph of your notice to see which one you got.

Either version arrives from the plan at least 60 days before Medicare starts, and the opt-out goes back to the plan by phone or in writing. Before the effective date, confirm that Jacobi or Weiler, whichever you actually use, and your pharmacy on White Plains Road are in the D-SNP’s networks.
