Bayside: D-SNPs, IB-Dual, and the “default enrollment” letter

Bayside is mostly single-family houses and low-rise co-ops, from the Bay Terrace complexes near the Cross Island Parkway down through Bell Boulevard to Oakland Gardens. Many residents are retirees who have lived here for decades, and Medicaid often enters the picture later in life, after a stroke, a fall, or a spouse’s dementia makes home care necessary. Some are newly on Medicaid managed care and are surprised to get a default enrollment letter at all.

This page is for Bayside residents in that position and for the adult children who drive in on weekends to go through the mail. It explains the letter, what IB-Dual and Medicaid Advantage Plus are, and the free counseling numbers that work from any Bayside phone. It decides nothing about your coverage and is not a government site.

Bayside is in Queens County, and Medicaid is administered citywide by the NYC Human Resources Administration (HRA). ZIP 11360 covers Bay Terrace and the north, 11361 covers central Bayside around Bell Boulevard and the LIRR station, and 11364 covers Oakland Gardens to the south (approximate; verify with USPS). The Nassau County line is a short drive east, but Bayside addresses are Queens County; write “Queens.”

The hospitals most Bayside residents use are in Flushing, west along Northern Boulevard: NewYork-Presbyterian Queens on Main Street and Flushing Hospital Medical Center on Parsons Boulevard. The nearest public hospital is NYC Health + Hospitals / Queens in Jamaica Hills. Each is a separate entry in a D-SNP’s Medicare directory, and so is every practice along Bell Boulevard.

The Bayside LIRR station on Bell Boulevard is the neighborhood’s only rail link, and it runs to Manhattan through Woodside, not through Jamaica. The Q12 and Q13 along Northern Boulevard reach Flushing; the Q31 from Bell Boulevard heads toward Jamaica.

Bayside letters often go to people who got Medicaid recently, while arranging home care, and who have had Medicare for years. Default enrollment is designed for people moving from Medicaid managed care onto Medicare for the first time, so it may not apply to you the same way. Read the letter to see whether it describes a new Medicare start date or a change to an existing plan, and ask HIICAP which situation you are in.

If the letter is a true default enrollment notice, it will name your Medicaid plan’s own D-SNP and an effective date at least 60 days out; the opt-out window generally closes the day before (confirm on your letter). Before that date, confirm your Flushing hospital and your pharmacy on Bell Boulevard or Northern Boulevard are in the plan’s Medicare network.
